Please read the following instructions carefully!  

The application form will allow you to upload supporting documentation at each stage of the application.  Once uploaded you can link the same document where ever it is relevant to your application.  Things to note regarding uploads:-

  • There is a limit of 25 MB per file
  • Allowed file types include all extensions of the following:- doc, xls, ppt, pdf, rtf, txt, jpg, png
  • Keep your file title simple and make sure there are no dots at the beginning of the file name or extra underscores 
  • Your uploaded files are available for selection in the 'choose file' list for all application sections.  
  • You can delete incorrect uploads and select another from the 'choose file' list or upload new files. Take care deleting uploaded files as the file will delete from all instances of where it has been linked. 

You do not need to complete your application in a single session. As part of the process you will set up an account which will allow you to save your application and come back to it later.  To navigate through the application, please use the navigation bars on the left hand side of the screen.

Once all your information is entered along with supporting documentation, your application is then able to be "submitted".  The application system will indicate any sections that require further completion in red.

Once your application is successfully submitted, it cannot be rescinded.  Please allow yourself enough time to check, save and submit your application.  

Please use a personal email address and not a DHB address.

The closing date for applications is Tuesday, 26 March 2024, 2:00pm NZ time

Referee Details

Please note that in 2024 NZAGS is collecting five (5) surgical/medical consultant referee reports, three (3) Allied Health Professsionals referee reports of which one must be a nurse and two (2) SMO doctors from non-surgical specialties referee reports
You must:
  • List five (5) referees who are surgical/medical consultants  
  • Ensure your surgical/medical referees supervised you in a clinical setting sufficiently to comment
  • Ensure your surgical/medical referees are from the past two years prior to closing date*
  • List three (3) referees who are allied health professionals of which one must be a charge nurse
  • Ensure your allied health professional referees are from the past two years prior to closing date*
  • List two (2) referees who are SMO doctors from non surgical specailties
  • Ensure your referees who are SMO doctors from non surgical specailties are from the past two years prior to closing date*
  • Seek prior permission and establish availability from all referees 

In addition for 2024

  • List all supervising consultants worked with for more than 3 months over the previous 2 years. You do not have to get permission from this group.

Do not list:

  • more than three (3) referees from any one rotation

*Applicants who have been undertaking a period of full-time study in a medically related discipline within the previous two years may extend that period by the period of the full-time study, up to a maximum of four (4) years.

You should:
  • Include work which is published or accepted for publishing, and only full articles from refereed journals or books.  Up to six (6) articles can be listed.
  • List authors in the order they appear in the publication
  • Include works relevant to surgery from the past five (5) years only 
Do not:

  • Include abstracts, letters or media releases
Acceptable documentary evidence includes:
  • First page of article clearly showing full reference

Acceptable documentary evidence includes:
  • Appropriate page of meeting programme
  • Letter of acceptance from meeting convenor identifying you as speaker
  • Abstracts selected to be presented at the NZAGS conference, to be held on 6 and 7 April 2024, will be accepted. Attach a copy of your acceptance letter.
